WASHINGTON -- U. S. health officials are looking into whether men who take Viagra or a placebo will have erectile dysfunction. And men who do, they say, have a more than five-fold increase in the risk of erectile dysfunction.
In some cases, they have been looking at other factors. Viagra, Pfizer's drug for erectile dysfunction, is only approved by the Food and Drug Administration for older men who use it and for people who have never had a heart attack. It also is approved for people who take the drug for a condition called angina, which is what occurs in the arteries that supply the heart.
Dr. William T. Dole, a cardiologist at the Cleveland Clinic, says his team is looking at other factors, such as blood pressure, cholesterol, diabetes, and other risk factors. And he says the results from the study may be better served by Viagra.
“We do know that Viagra, or sildenafil, is more likely to cause erectile dysfunction than placebo,” Dole said. “It’s important to remember that there are no randomized clinical trials that have demonstrated a relationship between Viagra and erectile dysfunction.”
But dole says the results may also be useful in men who do not have erectile dysfunction and want to take Viagra to get their blood to flow easily to the penis.
He says men who take Viagra, however, have a fourfold increase in the risk of developing erectile dysfunction than those who take a placebo. It's also more likely to develop low blood pressure and diabetes, he says.
Viagra, for its part, is a treatment for hypertension. It is also an anti-hypertensive. Doctors prescribe Viagra for people with a history of hypertension, angina, and heart problems. They use a drug called a beta-blocker to keep blood vessels open, which can help improve blood flow to the penis.
Viagra also may help prevent erectile dysfunction by helping to relax the muscles in the penis and improving blood flow to that area. But it is not a cure for all erectile dysfunction. So Viagra is not a permanent solution for erectile dysfunction, or an alternative treatment for the condition.
“I would be surprised if Viagra is still available,” said Dr. Robert A. Lue, who is also a cardiologist at the Cleveland Clinic.
Lue says doctors have been studying the effects of the drug for several years, but he says he has never tried to make any clinical judgment.
“I’m not saying that is a new drug,” he said. But he said the findings are “very reassuring.”
But Viagra and other similar drugs, such as sildenafil and tadalafil, do not work for everyone. There have been studies linking the two drugs together for erectile dysfunction, but none have been published on their side effects.
“The risk of side effects with sildenafil is very low,” Dr. Tadeep J. Khera, a urologist at Johns Hopkins Hospital, said. “This is a drug that was developed for patients and it has very few safety concerns. It’s the most common medication used to treat erectile dysfunction.”
Viagra also is not approved for treating pulmonary hypertension.
There have been studies showing that Viagra is better at helping men get or maintain an erection than sildenafil, a drug in which the drug relaxes blood vessels to allow blood to flow easily.
“The risk is very low,” Dr. Khera said. “The side effects are very low.”
E. J. Cramer, the head of urology at Johns Hopkins Hospital, said he had not tried Viagra. Viagra is “a very common drug for erectile dysfunction, and it’s been for a long time,” he said. The drug was approved for patients with erectile dysfunction in 1998.
There has been no official approval of sildenafil, or tadalafil, for pulmonary hypertension. The drug is approved for people who have low blood pressure or pulmonary hypertension.
“We do know that sildenafil is more likely to cause lower blood pressure and low blood pressure in people who have pulmonary hypertension,” Dr. Cramer said.
